Project title: Leveraging patient-reported data to address racial disparities in 30-day readmission in patients with cardiovascular disease
The goals of this research are to examine racial differences in self-perceived and actual risks of 30-day readmission among patients hospitalized with cardiovascular disease and to identify socioeconomic, psychosocial, behavioral, and health-related factors associated with these differences.
